2. YARD WASTE SITE PERMIT APPLICATION
   Please complete this application, print it and bring it to city hall along with $10.00 cash, check or card payment. You will be issued a yard waste site key card, which will allow you access into the locked yard waste site. The key card will be inactivated when the site closes December 1st and will need to be renewed for $5.00 to be re-activated for the following year. You may use one key card for two vehicles. Each household may purchase up to two permits at the cost of $10.00 each. One key card will be issued with each permit.
