    1. THIS APPLICATION IS FOR WAITE PARK RESIDENTS ONLY.
      APPLICANTS RESIDING OUTSIDE OF WAITE PARK MUST SUBMIT THE APPLICATION TO THEIR LOCAL LAW ENFORCEMENT AGENCY.
    2. This is used to complete a background check.
    3. This is not required, but may be requested at a later time to complete a background check.
